1.3. Know how to investigate accidents and incidents to establish causes

Issues:

While it is too late once there has been an accident, it is still important to establish its cause in order to prevent further accients. Incidents - where there is an occurence that could have resulted in injury, provide the best opportunities for investigation

Task:

1. Reporting Accidents

There are legal requirements for reporting accidents...outline the information that needs to be collected

2. Collect other relevant information

Collect further information (what/when/record book/stats) from other employees and employers..

Who can collect what? Joint investigation? legal constraints on all but HSE inspectors

3. Analyse cause of an accident/incident

Often hear during accident invetsigation that looking for 'cause' rather than 'blame'. Information so collected cannot be refused a claimant in a case. And with new 'blame' culture, how feasible is this?
